Membership Application and Payment
Welcome Members/Prospective Members. Below is an application for the 2016-17 membership year (July 1, 2016 – June 30, 2017) with two options: (1) You may print and mail in a form, or (2) you may complete a form online and pay here in two steps. We are at half-year time, so payment to June 2017 is only $15.
Note: PayPal costs about 3% of the total, so if you are making a large donation, please consider making your payment by check mailed to P. O. Box 423, Titusville, Fl 32781 or by cash at the next luncheon meeting.
For option (1), if you prefer not to apply online, please click the NAL Membership Application, print a paper version of the form, and mail it with a check payable to NALFL to: Treasurer, P. O. Box 423, Titusville, Fl 32781.
For option (2), to apply online please complete the form below. If you are renewing and have no changes to your contact information, check the box indicating that status and submit the form. Otherwise please complete all areas to the best of your ability, choosing the correct category for your personal participation. Note that the blocks with asterisks are mandatory.
Once you complete the application form, BUT PRIOR TO SUBMISSION, please submit payment via PayPal.
